Goals of Care
At the National University School of Health and Human Services, the mission of the Nurse Managed Clinic Interprofessional Education & Collaborative Practice (NUNMC-IPE/IPCP) Initiative in collaboration with community organizations, is to extend the reach of healthcare delivery to under-resourced, inner city residents. Through collaborative practice agreements, the NUNMC-IPE/IPCP adva

nced practice nurses work closely with an interdisciplinary team of health professionals including: physicians; healthcare administrators; clinical lab scientists; clinical affairs regulators; informatics specialists; data analysts; and, social work, integrative health and public health practitioners. The primary aim of this dynamic team of health professionals is to develop cost effective care delivery strategies for patients diagnosed with acute and chronic illnesses in the community-based setting. The objectives of the Clinic are to: screen, identify and provide health promotion/disease management services, foster community in- volvement in the health assessment and referral process, and provide culturally and linguistical- ly sensitive health promotion/disease management health education.

The NUNMC-IPE/IPCP Initiative is a unique approach to delivering healthcare that utilizes telehealthcare technologies to monitor clinical conditions on a daily basis in the home or community setting. This innovative, collaborative outreach Clinic, working in partnership with the UCLA Geffen School of Medicine and community-based organizations, is staffed by interprofessional healthcare professionals, who travel to under-resourced neighborhoods to provide primary preventive healthcare services for patients and their families. Health professionals representing the disciplines of nursing, medicine, social work, public health, informatics, clinical lab science, clinical regulatory affairs, data analytics, healthcare administration and integrative health collaborate in the delivery of care.

The utilization of this academic-community partnership approach blends the unique insights of community health leaders with more traditional academic evidence-based data, to provide healthcare services for under-resourced urban communities Care is provided at no out-of-pocket cost for the patients served. This initiative is expected to serve hundreds of residents in its first year of operation. The overall aim is to break down the financial, social, cultural, and transportation barriers that prevent patients from seeking proper healthcare, and to fill the gap in linking people in need with available resources.
