انقاذ ناجح!
تشكل الشرايين التاجية ذات التصلب المتقدم تحديا لجراح القلب لإجراء التوصيل الصحيح، مما يستلزم استئصال الجلطات التى تعوق سريان الدم داخل الشريان، الأمر الذى يضفى درجة أعلى من الصعوبة الجراحية لضمان سلامة الشرايين التاجية أثناء و بعد الجراحة. صورة لجلطة عملاقة تناهز ٥ سم طولا، تم استئصالها من الشريان الأمامى الأيسر للقلب أثناء جراحة لتوصيل الشرايين التاجية، أ.د. وائل حسنين. A successful rescue!
Advanced coronary atherosclerosis represents a challenge for the cardiac surgeon to achieve a sound anastomosis, necessitating removal of atheromas impeding normal flow within the artery, adding more to surgical difficulty to ensure the integrity of coronary arteries during and after surgery. يعد فرط تعرق اليدين أحد أكثر الإضطرابات تأثيرا على جودة حياة الفرد و شيوعا بين الأطفال و البالغين. يقدم قسم جراحة القلب و الصدر جراحة منظار الصدر ذات التدخل المحدود لاستئصال العصب السمبثاوى و علاج فرط التعرق بجروح لا تزيد عن ٢ سم، مما يساهم فى تحسين تعافى المريض و تقليل آلام ما بعد الجراحة و خروج المريض للمنزل فى يوم الجراحة دون الحاجة للبقاء فى المشفى.

Palmar hyperhiderosis is a common disorder affecting children and adults, causing a significantly negative impact on patients' quality of life. Cardiothoracic Surgery Department is offering minimally-invasive thoracoscopic sympathectomy surgeries with incisions less than 2 cm, which contributes to improving patients' recovery, reducing postoperative pain and discharging the patient at the same day of surgery.

افتتاح وحدة جراحة قلب و صدر الأطفال بمستشفى سموحة الجامعى هذا الأسبوع و إجراء عدة جراحات ناجحة تشمل تقشير للغشاء البلورى لمرضى الارتشاح البلورى الصديدى المزمن و اصلاح عيب خلقى بالقلب عن طريق ربط وصلة شريانية و استئصال فص بالرئة ناتج عن التهاب صديدى مزمن بالشعب الهوائية. تشمل الوحدة ١٣ سرير و عناية مركزة جراحيه و غرف عمليات مجهزة حديثا. يتطلع القسم إلى إجراء المزيد من الجراحات المتقدمة للأطفال و توفير خدمة متميزة لقطاع كبير من المرضى.

Inauguration of Pediatric Cardiothoracic Surgery Unit this week and performing several successful surgeries including decortication for chronic empyema, patent ductus arteriosus ligation and lobectomy for chronic bronchiectasis. The unit has a capacity of 13 inpatient beds as well as state-of-the-art pediatric surgical intensive care unit and operation theaters. The Departmemt is looking forward to performing more advanced surgeries for children and providing an exceptional service for a wider sector of patients.

Surgeons' debate: What is the optimal anticoagulant to prescribe to pregnant women with mechanical heart valves?

- There is 'no' optimal anticoagulant protocol for pregnant women with mechanical heart valves.
- Vitamin K antagonists (VKAs) e.g. warfarin are the safest for the mother, but associated with fetal complications and least live births. Heparins are associated with more maternal complications and require costly monitoring.
- Sequential strategy may be adopted, where unfractionated heparin (UFH) or better, low-molecular weight heparin (LMWH) may be administered in the first trimester, followed by VKAs in the second and third trimester, especially when target international normalized ratio (INR) can be achieved with doses less than 5 mg/ day.

تجرى جراحات التدخل المحدود لأمراض الصدر للعديد من الحالات عن طريق مناظير الصدر مما يساهم فى صغر الجرح و سرعة شفاء المرضى (إصلاح تعرق اليدين عن طريق المنظار، د. وليد أبو عرب).
Minimally-invasive thoracic surgery is being done to more patients nowadays by thoracoscopes, contributing to smaller incisions and faster recovery (thoracoscopic sympathectomy for treating primary hyperhidrosis, Dr. Walid Abu Arab).

تتم أغلب جراحات القلب عن طريق 'ماكينة القلب و الرئة الصناعية' التى تعمل كمضخة لتزويد الجسم بالدم أثناء توقف القلب عن العمل. (صورة لإحدى ماكينات القلب قبيل إجراء الجراحة لإصلاح ثقب بالقلب، المستشفى الجامعى الجديد، الإسكندرية).
Most of cardiac surgeries are accomplished using 'heart-lung machines', acting as pumps that provide the body with blood during cardiac arrest.
