The 50th Hong Kong Sevens Tournament was held from 17 – 19 April 2026, Hong Kong hosted the first leg of the three round Rugby Sevens World Championship. The University of Hong Kong’s Department of Emergency Medicine and Gleneagles Hospital were well represented. Professor Timothy Rainer was the Tournament Medical Director and Match Day Doctor providing Head Injury Assessment (HIA) services at the Pitchside and in the HIA room. Professor Arthur Cheung was the Event Doctor of the Medical Team providing immediate medical care for players in the medical room. Apart from the common injuries like facial laceration, rib or shoulder contusion and knee sprain, there was significant trauma like fracture dislocation of anklesrequiring urgent transportation to designated hospital for operation.

The Hong Kong Sevens International Sports Medicine Conference was held on 14th and 15th April 2026 in the Kai Tak Sports Park. Professor Timothy Rainer was the moderator on the track: INJURY MANAGEMENT: RETURN-TO-PLAY & PERFORMANCE. The conference also covered hot topics in sports, e.g. female athlete health and performance; brain health for athletes. Several workshops provided interactive discussions on female health in sports, head injury assessment and hands-on sessions for sports taping.

Through the insightful sharing and engaging discussion, the audience gained valuable perspectives on China’s 15th Five-Year Plan and its implications for the Greater Bay Area and Hong Kong.

Real-world data (RWD) research extends well beyond simple retrospective analyses. Traditional designs—cohort, case–control, and cross-sectional studies—remain central, but now sit alongside pragmatic clinical trials (PCTs) and single-arm studies, broadening how we generate evidence in routine care.

Cohort studies track how exposures influence outcomes over time; case–control studies are suited to rare outcomes; cross-sectional studies offer a snapshot of disease patterns.

PCTs evaluate interventions under everyday clinical conditions, providing more generalisable evidence. Single-arm studies, often used in oncology, compare new treatments with historical controls when randomisation is not feasible.

Across all designs, appropriate choice, methodological rigour, and ethical conduct are essential.

很多醫生一談及真實世界資料(RWD)研究,往往將其等同於「回顧分析」或一般「觀察研究」。其實,RWD之研究設計遠較此為豐富多樣。當前主流固然仍以觀察研究為核心,如隊列研究、病例對照研究與橫斷面研究,但隨著方法學的發展,實用臨床試驗(pragmatic clinical trial, PCT)與單臂試驗(single arm trial)亦逐漸納入RWD的範疇。

在觀察研究中,若探討某種藥物的長期療效,隊列研究為首選,最能清楚呈現由「因」至「果」的過程;若研究對象為罕見疾病,或研究的結果已然發生(如特定不良反應),則病例對照研究更為有效;至於橫斷面研究則宛如一張「快照」,適於描繪疾病在某一特定時間的發病率與特徵。

實用臨床試驗,雖仍屬試驗,卻更貼近實際日常臨床情況,納入標準更為廣泛,干預措施亦極具彈性,因此所得的數據更具有「真實」說服力。此外,在腫瘤等特定領域,對於缺乏有效治療手段的疾病,則可採用單臂試驗,不設對照組,僅觀察試驗組療效,並可以外部歷史數據作為對照(External Control)。

至於應選擇那種設計,關鍵在於研究問題的研究題材以及手中數據基礎是否足以支撐;理想的研究設計,必須在現有數據的下,最大可能貼近事實真相,同時嚴格遵守科學原則與倫理規範。

We are pleased to mark the commencement of the new MBBS Year V/VI Assistant Internship curriculum. Yesterday, in collaboration with the HKUMed Admissions Team, we hosted the first session at the HKU Medical Campus. A group of 120 high school students attended and had the opportunity to hear from our Department’s Clinical Associate Professor, Dr Abraham Wai, together with Year V/VI MBBS students, who shared their knowledge and insights into Emergency Medicine.

The primary and clinically rich data source is Hospital Information System (HIS) data, particularly Electronic Medical Records (EMRs), which encompass admission notes, prescriptions, and surgical records essential for reconstructing a patient's diagnostic and treatment pathway. Additionally, medical insurance payment data serves as a vast repository, linking medical behaviours and costs, making it invaluable for health economics research and healthcare resource utilization analysis.

Two other significant data sources include Registry data, organized for specific diseases (like cardiovascular disease or diabetes) or (like pregnant women), which ensures high quality, and Natural Population Cohort data, which tracks healthy populations over time, providing critical insights for disease prevention and early diagnosis.

Relying on a single data source often falls short; for example, hospital data may overlook post-discharge outcomes, while insurance data may lack lab results. Thus, integrating multiple data sources is increasingly favoured to create a comprehensive view of the patient, with the choice of data contingent upon research objectives.

醫院訊息系統(HIS)數據,尤以電子病歷(EMR)為最核心且信息豐富的來源,包含入院記錄、病程日誌及手術記錄等,為還原患者醫療歷程提供第一手資料。此外,醫療保險支付數據亦作為重要資源,不僅紀錄診療行為以及相關費用,適合醫健經濟學及醫療資源研究。

值得關注的還有兩類重要數據源:登記(Registry)數據,專為特定疾病(如心血管病、糖尿病)或人群(如孕婦)建立,具高結構化及可靠的優勢;能透過長期追蹤提供疾病預防與早期診斷的佐證。

在研究實踐中,僅依賴單一數據來源往往不足以滿足需求。例如,醫院數據或許無法揭示出院後結果,醫保數據則可能缺乏具體檢測指標。因此,多源數據融合成為趨勢,將臨床數據、醫保數據及社區衛生中心數據整合,能構建全面的患者全視圖,確保研究的可靠性與有效性。選擇哪種數據源,完全取決於你的研究目的,切記「巧婦難為無米之炊」,數據源的可靠性直接決定了你研究結果的可信度。

Randomised Controlled Trials (RCTs) are like fishing in a controlled pond; Real World Data (RWD) studies observe fish in real rivers and oceans. RWD are data on patient health and care collected in routine practice, not strict clinical trial conditions. They overcome many limits of RCTs by including broader populations (the elderly, children, patients with comorbidities) and longer-term treatment outcomes. RWD come from Electronic Medical Records (EMRs) and Hospital Information Systems (HIS), medical insurance settlement records, drug usage records, and even wearables.

For clinicians, this is a potential “gold mine” that answers questions RCTs can’t, such as long-term safety, cost-effectiveness, and epidemiology. But its value depends on data quality, rigorous methods, and starting with a clear clinical question.

隨機對照試驗(RCTs)如在理想魚塘垂釣,真實世界數據研究 (RWD)則如觀江河之魚。RWD是日常醫療實務中病人的健康與醫療服務紀錄,打破了傳統研究的限制,更廣泛涵蓋老幼與多重共病等人群,並呈現長期療效。資料來源包括電子病歷、醫院信息系統﹑醫保結算記錄、藥品使用記錄,甚至穿戴裝置。

對於臨床醫生而言,RWD既為新興途徑,亦能補足RCT於長期安全性、成本效益與流行病學所難及之處。然而RWD的價值繫於數據品質與方法嚴謹;必先明定臨床問題,方可從數據中提取支持診療決策。
