INFORMED CONSENT

This is an informed consent form for students who wish to participate in an exit interview and allow the data gathered through the Google form to be used for university inventory, research, and other activities. Before proceeding with the interview and submitting your data, please read this form carefully and make sure that you fully understand the information provided.

Purpose:
The purpose of this exit interview is to gather information about your experience as a student in our university. The information you provide will be used to identify areas of strength and improvement for the university, which will help us enhance the quality of our educational programs and services.

Procedure:
If you choose to participate in the exit interview, you will be asked to complete a Google form that will ask for your feedback on various aspects of your university experience. The form will be sent to you via email and can be completed at your own convenience. The form will take approximately 10-15 minutes to complete.

Confidentiality:
Your participation in the exit interview is voluntary, and all data collected will be kept strictly confidential. The data will be used for university inventory, research, and other activities only, and will not be shared with any third parties without your consent. Your name and identifying information will not be included in any reports or publications resulting from this study.
