Please be advised that PSU-LHS will not accept transferees for Grades 8 to 10 (Junior High School) and Grade 12 (Senior High School) for this academic year.
๐๐ฝ๐ฝ๐น๐ถ๐ฐ๐ฎ๐ป๐๐ ๐ฎ๐ฟ๐ฒ ๐ฟ๐ฒ๐พ๐๐ถ๐ฟ๐ฒ๐ฑ ๐๐ผ ๐ฝ๐ฟ๐ฒ๐ฝ๐ฎ๐ฟ๐ฒ ๐ฎ๐ป๐ฑ ๐๐๐ฏ๐บ๐ถ๐ ๐๐ต๐ฒ ๐ณ๐ผ๐น๐น๐ผ๐๐ถ๐ป๐ด ๐ฑ๐ผ๐ฐ๐๐บ๐ฒ๐ป๐๐ ๐๐ฝ๐ผ๐ป ๐ฎ๐ฝ๐ฝ๐น๐ถ๐ฐ๐ฎ๐๐ถ๐ผ๐ป:
1. Photocopy of Form 9/Form 138 (Report Card) reflecting grades from the first to the third quarter.
2. Two (2) pieces of 2x2 ID photos, taken with a white background and bearing a name tag.
3. Photocopy of the Certificate of Good Moral Character, issued by the applicantโs current school.
4. One long brown envelope, properly labeled with the applicantโs full name in the format: Surname, Given Name M.I. and indicating the grade level being applied for.
๐ง๐ผ ๐ฐ๐ผ๐บ๐ฝ๐น๐ฒ๐๐ฒ ๐๐ต๐ฒ ๐ฎ๐ฝ๐ฝ๐น๐ถ๐ฐ๐ฎ๐๐ถ๐ผ๐ป ๐ฝ๐ฟ๐ผ๐ฐ๐ฒ๐๐, ๐ฎ๐ฝ๐ฝ๐น๐ถ๐ฐ๐ฎ๐ป๐๐ ๐บ๐๐๐ ๐ณ๐ผ๐น๐น๐ผ๐ ๐๐ต๐ฒ๐๐ฒ ๐๐๐ฒ๐ฝ๐:
1. Secure an application form from the Guidance and Admission Office.
2. Proceed to the cashierโs office to pay the examination fee.
3. Submit the completed application form along with all required documents to the Guidance and Admission Office. Upon submission, applicants will receive their examination permit.
4. Stay updated by following the official page of PSU Bayambang, where the list of qualified applicants will be posted.
