1) All CTU CCMSC enrollment qualifiers for 1st Semester AY 2023-2024 who are now enrolled are required to fill out this form which will serve as your admission data in the office and for admission billing purposes to be submitted to the DBM.

2) All old students for readmission (e.g., transferees from other CTU campuses, shifters, and returnees) who applied in the admission portal this year and who are now enrolled are also required to fill out this form.

This form will only accept responses until 𝗦𝗲𝗽𝘁𝗲𝗺𝗯𝗲𝗿 𝟮𝟰, 𝟮𝟬𝟮𝟯, 𝗮𝘁 𝟭𝟭:𝟰𝟱 𝗽.𝗺.
