Requirements for First Year Students

1. Notarized Parental Consent (Yellow Form)
2. Random Drug Testing Form (Green Form)
3. Contract of Compliance

Note: Those who have already submitted the above-mentioned forms are advised to wait for their clearance to be processed and cleared.

Requirements for Second, Third, and Fourth Year (Non-Graduating) Students

• No requirements needed.
• Please wait for your clearance to be processed and cleared. There is no need to visit the Office of Student Affairs (OSA).

The Student Online Profile (SOP) is now open.

All Possible Candidates for Graduation – Batch 2026 are required to submit their Student Online Profile (SOP).

𝐌𝐀𝐍𝐃𝐀𝐓𝐎𝐑𝐘 𝐈𝐍𝐒𝐓𝐑𝐔𝐂𝐓𝐈𝐎𝐍𝐒:
1️⃣ Complete the Application for Graduation (AFG) link posted by the Registrar before filling out the SOP.
2️⃣ A day after submitting the AFG, your name will appear in the SOP system search names, allowing you to complete the SOP.
3️⃣ Submit the SOP in complete to access your pictorial appointment schedule.

𝐒𝐓𝐑𝐈𝐂𝐓 𝐑𝐄𝐌𝐈𝐍𝐃𝐄𝐑𝐒:
1️⃣ Steps must be followed exactly. All information must be accurate and complete. Final and irrevocable.
2️⃣ No SOP, no pictorial.
3️⃣ Late submissions will not be accommodated, as the SOP system closes promptly at 11:59 PM on April 17, 2026.
⚠️ Failure to comply will esult in exclusion from the pictorial and Batch 2026 yearbook.
