Proud to have been one of the authors of the Philippine Clinical Practice Guideline for Periodic Health Examination: Screening for Neoplastic Diseases, now published in Acta Medica Philippina.

As we continue to emphasize early cancer detection, I hope this guideline helps promote evidence-based and appropriate cancer screening in the Philippines.

BONE METASTASES IN CANCER

"Kumalat na po sa buto ang cancer ninyo..."

Cancer can spread to the bones when cancer cells travel through the blood and settle in the bone.

Cancer cells can disturb the normal balance between:
osteoclasts — cells that break down bone
osteoblasts — cells that build bone

When this balance is disrupted, bones can become weak, painful, or prone to fractures.

Bone metastases commonly come from:
✔️Breast cancer
✔️Prostate cancer
✔️Lung cancer
✔️Kidney cancer
✔️Thyroid cancer
✔️Multiple myeloma

Signs and Symptoms
✔️Bone pain, often persistent or worsening
✔️Back pain
✔️Fractures with minimal trauma
✔️Weakness or numbness if the spine is compressed
✔️High calcium levels causing nausea, constipation, confusion, thirst, or weakness

How to Diagnose
✔️X-ray
✔️Bone scan
✔️CT scan
✔️MRI, especially for spine symptoms
✔️PET/CT in selected cases
✔️Blood tests such as calcium, kidney function, and alkaline phosphatase

GRAND ROUNDS ON IMMUNOTHERAPY AND MYASTHENIA GRAVIS

Immunotherapy has revolutionized cancer treatment by helping the body's immune system recognize and attack cancer cells. However, in rare cases, it can also cause the immune system to attack normal tissues, leading to immune-related adverse events (irAEs).

One rare but potentially serious complication is Myasthenia Gravis (MG).

🔹 What is Myasthenia Gravis?
Myasthenia gravis is an autoimmune disease where the immune system attacks the communication between nerves and muscles, causing muscle weakness.

Symptoms may include:
✔ Drooping eyelids
✔ Double vision
✔ Difficulty speaking or swallowing
✔ Weakness of the arms and legs
✔ Shortness of breath

🔹 Why is this important in cancer patients? Although uncommon, immunotherapy drugs such as pembrolizumab, nivolumab, durvalumab, and others can trigger new-onset myasthenia gravis or worsen previously diagnosed disease.

Early recognition is critical because severe cases can progress to a myasthenic crisis, a life-threatening condition that affects breathing.

💡 If you are receiving immunotherapy and develop new muscle weakness, drooping eyelids, difficulty swallowing, or breathing problems, inform your oncology team immediately.

Immunotherapy saves lives—but awareness of its rare complications helps keep patients safe.

It's TULI SEASON!

My son asked me if circumcision will lead to less risk of pe**le cancer.

The answer is YES!

Circumcision is associated with a lower risk of pe**le cancer, especially when performed in infancy or childhood.

The protective effect is thought to be due to:
1.) Improved ge***al hygiene
2.) Lower risk of chronic inflammation
3.) Lower rates of phimosis
4.) Reduced persistence of high-risk Human Papillomavirus infection

However, circumcision does not completely prevent pe**le cancer since there are other risk factora. Please see poster below for more details.
