One chapter in our recently-released publication, Ethical and Clinical Complexities in the Treatment of Schizophrenia: Liberty or Life, discusses an innovative program at the Los Angeles County jail that showcases the transformative impact of compassionate care provided by inmate Mental Health Assistants (MHAs) for incarcerated patients with serious mental illness. The MHAs undergo extensive training in trauma-informed care and work to bridge the communication gap between patients and clinical staff. The chapter outlines the philosophy, structure, and goals of the program, emphasizing the need for a restorative environment and meaningful services within the jail, and can serve as a model for other carceral settings.

Schizophrenia is a chronic and severe mental disorder that affects approximately 1% of the global population. Characterized by profound disruptions in thinking, perception, and behavior, it often leads to significant impairments in social and occupational functioning. Traditionally, schizophrenia has been associated with a deteriorating course; however, contemporary research underscores the potential for improved outcomes through early intervention strategies. This article explores the rationale, components, and benefits of early intervention, emphasizing its critical role in enhancing prognosis and recovery.

The following clinical vignette illustrates the value of early intervention and the consequences of missed opportunities for treatment earlier in the progression of symptoms.

What role did serious mental illness play in Jackson Po***ck’s drip paintings?

The link between creativity and serious mental illness (SMI) is widely discussed. Jackson Po***ck is one example of a giant in the field of art who was both highly creative and experiencing an SMI. Po***ck created a new genre of art known as abstract expressionism (“action painting”) defined as showing the frenetic actions of painting. The question arises whether his SMI played any role in the way he created his drip paintings, especially when he was overactive and manic.

Over the decades, research has demonstrated that Conditional Release Programs (CONREP) and Compulsory community treatment can reduce recidivism among forensic patients discharged from inpatient commitment. This study synthesizes current knowledge—including findings from a 2024 California Department of State Hospitals report—to evaluate the impact of involuntary community treatment on recidivism and patient outcomes. Anosognosia, commonly understood as a lack of insight, renders individuals with schizophrenia and schizoaffective disorder unable to understand that they are living with a disease, often resulting in a refusal to accept treatment. This article examines the existing legal theories of competency and informed consent and proposes a statutory definition of competency that encompasses the specific needs of people with anosognosia, while considering the significant interests at stake when taking away an individual’s right to choose or refuse treatment.

Treatment of patients with schizophrenia in the Austrian healthcare system is generally voluntary. This applies both to outpatient care provided by specialists in private practice, hospital outpatient clinics, or social psychiatric outpatient clinics and to inpatient care in hospitals. However, there is an exceptional situation in which the patient’s freedom of will is restricted by law. This is the case when acute danger to self or others caused by the disorder is present. With the involvement of the district court, the patient advocate, a possible adult representative, and an external expert, the patient’s freedom of movement can be restricted for a certain period of time to enable treatment.

People with serious mental illness (SMI) are over-represented throughout the US criminal justice system. To address this issue, forensic assertive community treatment has recently emerged as a best-practice intervention. Also known as forensic ACT, ForACT, or most commonly as “FACT,” forensic assertive community treatment is an adaptation of the assertive community treatment (ACT) model. Unlike ACT, however, FACT is purposefully designed to prevent arrest and incarceration among people with SMI who have histories of involvement with the criminal justice system.
