ANSWER to Tuesday's NCLEX question:
(sorry this is a day late!)

The nurse is planning discharge teaching for a client with Parkinson's disease. To maintain safety, the nurse should make which of the following suggestions to the family?

1) Install a raised toilet seat - CORRECT - helps client to be independent; slightly elevate the back leg of chairs
2) Obtain a hospital bed - Incorrect: no indication that this is needed
3) Instruct the client to hold his arms in a dependent position when ambulating - Incorrect: should swing arms to assist in balance when walking
4) Perform an exercise program during the late afternoon- Incorrect: activities should be scheduled for late morning when energy level is highest and client won't be rushed

Do you remember the NCLEX question? Here's the answer and rationale.
A 40-year-old, G7, P4, A2, 36 weeks pregnant woman was admitted because of complaints of vaginal bleeding. She also noticed a lack of fetal activity in the 3 days prior to admission. The nurse's first action is to

A. check for fetal heart tones - CORRECT

B. ask for ultrasound examination

C. ask for x-ray of the abdomen

D. palpate for fetal movement

Explanation

•A. Finding the fetal heart tones is the easiest way to check the status of the pregnancy in this situation, and should be done first.

• B. If no fetal heart tones are noted, a sonogram should be ordered to confirm fetal status.

• C. Incorrect: An x-ray of the abdomen will not assess whether the fetus is alive or not.

•D. Incorrect: There will be no fetal activity if the fetus dies in utero, but palpation is not a diagnostic test.
