A message from University Health Services at UW-Madison(UHS):

As we prepare to welcome students to campus, it’s important that your student shares their vaccination records through the secure MyUHS portal. If a vaccine-preventable illness like measles occurred on campus, state public health laws require students to quarantine if we don't have proof of immunity.

For more information on medical clearances and instructions on how to upload vaccination records, please visit: https://www.uhs.wisc.edu/about/medicalclearance/

*If you are from Wisconsin, your student can check the Wisconsin Immunization Registry for their vaccination records. If you are from another U.S. state, navigate to your state’s immunization website using this portal. You may also contact your primary care provider to obtain your student’s records.

This fall, University Health Services (UHS) is hosting flu and COVID-19 vaccination clinics for students across campus. As we head into cold and flu season, vaccination is one of the best strategies to prevent the spread of illness. When encouraging your student to get vaccinated, please make sure they have their health insurance information and Wiscard for registration.
