You are invited to join Clark County Extension in a Planning AHEAD class that will be hosted at Colby Senior Care.
Planning AHEAD is an end-of-life planning curriculum for all ages developed by UW–Madison Division of Extension faculty and staff. This 7-session research-based program will help participants understand how to prepare for the end of life for themselves or a loved one, and why it’s important to have a plan in place.

This program and workbook were designed for residents in Wisconsin and is specific to Wisconsin law. If you are not a resident of Wisconsin, please refer to your state’s laws around Advanced Medical and Legal Directives, and Estate Planning.

• Discuss the intersection of connectivity and quality care, identifying how expanded broadband can transform health services and patient experiences.

• Identify opportunities and gaps in connected care, shaping regional priorities for innovation and investment.

• Collaborate on practical solutions to address healthcare service gaps and ensure equitable access to care.

Planning AHEAD is a seven session holistic end-of-life planning program that will guide participants through Advance directives, Handling financial changes, Estate Planning, and Arriving at Decisions for the end of this life. Participants will receive a digital or hard copy of the program workbook (included in $30 registration fee).
