If you live in a rural area, and are living with heart failure or assisting someone who is, please call or complete the online interest form (scan the QR code on this flyer to access the form). Dr. Lucinda J. Graven, PhD, APRN, FAHA is currently enrolling heart failure patients and their care partners who live in rural areas for a study to learn more about:

1. Challenges that you and your care partner experience related to heart failure.
2. Strategies that may be useful in helping individuals with heart failure and their family care partners who live in rural areas manage challenges associated with heart failure self-care.

This study will enroll “care teams’ which include the individual with heart failure and their family care partner. Participation in this study DOES NOT involve travel to meet with research staff. You will be asked to participate in 1 interview over the telephone as part of the study that will be scheduled at your convenience.

Compensation of $50 per care team who complete a 1-time individual telephone interview will be given.
