⚠️ THE HIDDEN DANGER OF COMMON PAINKILLERS IN PREGNANCY ⚠️

A newborn baby was rushed to the Neonatal Intensive Care Unit (NICU) just five hours after birth, struggling to breathe. The cause? A common painkiller the mother had taken during pregnancy.

1️⃣ How does a baby’s heart work before birth?

Before birth, a special blood vessel called the ductus arteriosus keeps blood flowing properly in the baby's heart.

This vessel is meant to stay open until after birth when the baby starts breathing on its own.

2️⃣ What went wrong?

The mother took Brufen (ibuprofen) to relieve a respiratory infection for 5 days before delivery.

This caused the ductus arteriosus to close too early, putting extreme stress on the baby’s heart and lungs.

As a result, the baby struggled to breathe and needed urgent medical support.

3️⃣ How was the baby saved?

Doctors provided oxygen support and CPAP (a special breathing machine) to reduce stress on the heart.

Over time, the baby's heart recovered, and the newborn regained normal health.

Key Lesson for Expectant Mothers:

BEFORE YOU CONSIDER CLOMIPHENE, READ THIS!

Starting Clomiphene requires proper counseling. It’s essential to know that using this medication significantly increases the chance of multiple pregnancies (e.g., twins, triplets, or more). Women on Clomiphene are 8-10 times more likely to have twins than those who ovulate naturally.

What Is Clomiphene?

Clomiphene is a prescription medication used to treat certain types of female infertility, such as polycystic o***y syndrome (PCOS). It is typically prescribed by a gynecologist or fertility specialist and should be taken under strict medical supervision, with the necessary fertility tests performed beforehand. These tests help determine whether infertility is due to a lack of ovulation.

Approximately 80% of women who take Clomiphene will ovulate, and 20-30% may become pregnant, depending on individual factors like age, weight, ovarian reserve, and other health conditions.

How Does Clomiphene Work?

Clomiphene is an antiestrogenic drug, meaning it blocks estrogen receptors. This mechanism "tricks" the body into thinking estrogen levels are lower than they are, prompting the brain to release more follicle-stimulating hormone (FSH) and luteinizing hormone (LH). FSH stimulates the ovaries to produce egg follicles, which mature and are then released as eggs during ovulation. LH then signals the ovaries to release these eggs.

While a typical menstrual cycle releases only one egg per month, Clomiphene often causes the ovaries to release two or even three eggs, increasing the likelihood of multiple births.

How Is It Taken?

For women who menstruate but do not ovulate, Clomiphene is usually started at 50 mg on the second, third, or fifth day of their cycle and taken daily for five days. Ovulation typically occurs 7-10 days after the last pill. For best results, couples are advised to have in*******se every day or every other day from Day 11 through Day 21. Case Scenario: My Medication Therapy Review with Mr Mwaba(not real name) Taking Amlodipine and Simvastatin

Background: Mr. Mwaba, a 55-year-old man from Chimwemwe, has been taking Amlodipine (for high blood pressure) and Simvastatin (for high cholesterol) for the past year. He recently visited me for a Medication Therapy Review (MTR) as part of his medication therapy management (MTM) program.

My Review Process:

Step 1: Reviewing Mr. Mwaba Medications

Amlodipine (5 mg once daily): A calcium channel blocker to manage blood pressure, reducing the risk of heart disease and stroke.

Simvastatin (40 mg once daily): A statin to help lower cholesterol, also reducing the risk of heart attacks and strokes.

Step 2: Checking for Potential Drug Interactions As I reviewed Mr. Mwaba's medications, I noticed a potential interaction between Amlodipine and Simvastatin. Amlodipine can increase Simvastatin levels in the body, which may raise the risk of side effects like muscle pain or weakness (a condition called rhabdomyolysis).

Step 3: Consulting with Mr. Mwba To understand any health changes, I asked Mr. Mwaba if he had noticed symptoms like:

1. Muscle pain or weakness

2. Unusual fatigue

3. Any changes in blood pressure or cholesterol

Mr. Mwaba mentioned he was feeling well overall, though he had noticed occasional muscle soreness, particularly after physical activity, which he’d assumed was normal.

Step 4: Making Recommendations After discussing the potential interaction, I recommended the following:

1. Lowering the Dose of Simvastatin: I suggested reducing Simvastatin from 40 mg to 20 mg to decrease the risk of muscle issues, following guidelines that limit Simvastatin to a maximum of 20 mg when taken with Amlodipine.

2. Monitoring for Side Effects: I advised Mr. Mwaba to keep an eye out for muscle pain, weakness, or dark-colored urine and to contact his doctor right away if these symptoms appeared.

OVULATION PAIN

Facts about Ovulation Pain

1. Females aren't pretending about it.

It is Real Pain: Ovulation pain is a real experience for some women, not just imagined.

2. Not all Females experience it, and intensity varies

3. Similar to Menstrual Pain: It can resemble menstrual cramps, sometimes stronger, but usually mild and short-lived (24 hours).

4. Mittelschmerz is the medical term for ovulation pain.

5. Ovulation pain RESULTS when the egg leaves the egg basket in the ovaries and some fluids in the egg basket going along with it...
The eggs leaving the egg basket can cause pain... The fluid in the egg basket can irritate the lining of your tummy and cause this pain

You may also have fever, back pain and nausea

6. Many ladies get H***Y during ovulation. It is perfectly normal... You need to know the levels of hormones at the point of Ovulation... High!

You are not going to Hell fire if you feel this way

7. Not having ovulation pain does not mean you don't ovulate

"PHARMACY AND YOU"- ADHERENCE TO ANTI-HYPERTENSIVE MEDICATION, AN ESCAPE FROM COMPLICATIONS

The normal physiological function of the body requires blood circulation which is vital in enhancing the subsequent circulation of oxygen and other nutrients to the rest of the body. This circulation is supposed to be done at a certain blood pressure (BP), currently the pressure from 110/70mmHg and the upper limit of less than 130/80mmHg being the acceptable normal. Consistent increase above the higher normal limit is considered as hypertension which if not well controlled can cause complications like stroke, cardiac (Heart) failure, renal (Kidney) failure and damage to blood vessels of the retina which can cause vision problems.
First of all, it is worthy appreciating that under normal circumstances, the body is able to control its BP using the in-built mechanisms. Patients with stage 1 (mild) hypertension may therefore, be treated with non-pharmacological or supportive treatment involving the life style modification like moderation in fat and salt intake, regular exercising, having enough rest, stopping of smoking, reduction in alcohol intake and control of body weight without taking any medication. This supportive treatment approach is taken while hoping that the in-built systems will respond and do a re-setting to normal or controlled BP. If after taking all these steps, the BP still remains high, then the body is failing to control its BP and may need assistance and this is where anti-hypertensive drugs are finally prescribed.
Adherence to treatment which is taking medication correctly in terms of doses, number of times in a day and consistence all through the days has proved effective with evidence of reduced risk of associated complications while enabling patients to lead good, quality and productive lives without fear of end organ damage or sudden death.
However, poor adherence has been reported where many patients resort to stopping medication after normalization
