| 𝗖𝗮𝗹𝗹 𝗳𝗼𝗿 𝗣𝗵𝗶𝗹𝗛𝗲𝗮𝗹𝘁𝗵 𝗬𝗔𝗞𝗔𝗣 𝗦𝗲𝗿𝘃𝗶𝗰𝗲 𝗣𝗿𝗼𝘃𝗶𝗱𝗲𝗿𝘀
The Faculty Association of SLSU (FA of SLSU), SLSU Employees Association (SLSU EA), and SLSU Faculty and Employees Club (SLSU FEC) invite qualified and duly accredited healthcare providers to submit proposals for the provision of healthcare services under the 𝗣𝗵𝗶𝗹𝗛𝗲𝗮𝗹𝘁𝗵 𝗬𝗮𝗺𝗮𝗻 𝗻𝗴 𝗞𝗮𝗹𝘂𝘀𝘂𝗴𝗮𝗻 (𝗬𝗔𝗞𝗔𝗣) 𝗣𝗿𝗼𝗴𝗿𝗮𝗺 for eligible faculty and employees of Southern Luzon State University (SLSU).
This initiative aims to strengthen access to primary healthcare services, promote preventive health and wellness, and facilitate the availment of eligible healthcare benefits under the PhilHealth YAKAP Program. Through this undertaking, the organizations seek to identify a reliable, accessible, and responsive healthcare provider capable of meeting the primary healthcare needs of eligible members of the SLSU community in accordance with applicable PhilHealth policies, accreditation standards, and program guidelines.
📍𝗘𝗹𝗶𝗴𝗶𝗯𝗶𝗹𝗶𝘁𝘆 𝗥𝗲𝗾𝘂𝗶𝗿𝗲𝗺𝗲𝗻𝘁𝘀
Interested healthcare providers must be duly licensed or authorized to operate, as applicable, and hold valid PhilHealth accreditation or meet the applicable accreditation requirements for participation in the YAKAP Program.
Applicants shall submit the following documentary requirements, as applicable:
1. Valid business or mayor’s permit;
2. Securities and Exchange Commission (SEC) registration or other applicable registration documents;
3. Valid PhilHealth accreditation or proof of eligibility to participate in the YAKAP Program;
4. Relevant licenses and permits from the appropriate government regulatory agencies;
5. Tax clearance and other applicable compliance documents; and
6. Other documents necessary to establish the provider’s qualifications and capacity to deliver the required healthcare services.
📍𝗣𝗿𝗼𝗽𝗼𝘀𝗮𝗹 𝗥𝗲𝗾𝘂𝗶𝗿𝗲𝗺𝗲𝗻𝘁𝘀
Interested service providers shall submit a comprehensive proposal containing the following information:
▪️𝗣𝗿𝗼𝗳𝗶𝗹𝗲 𝗮𝗻𝗱 𝗾𝘂𝗮𝗹𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻𝘀 – Background of the healthcare facility, relevant experience, and proof of compliance with applicable regulatory and accreditation requirements.
▪️𝗦𝗰𝗼𝗽𝗲 𝗼𝗳 𝘀𝗲𝗿𝘃𝗶𝗰𝗲𝘀 – Detailed description of the primary healthcare services available under the PhilHealth YAKAP Program, consistent with current PhilHealth guidelines.
▪️𝗙𝗮𝗰𝗶𝗹𝗶𝘁𝗶𝗲𝘀 𝗮𝗻𝗱 𝗵𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲 𝗽𝗲𝗿𝘀𝗼𝗻𝗻𝗲𝗹 – List of available facilities, medical equipment, healthcare professionals, and other resources necessary for service delivery.
▪️𝗔𝗰𝗰𝗲𝘀𝘀𝗶𝗯𝗶𝗹𝗶𝘁𝘆 𝗮𝗻𝗱 𝘀𝗲𝗿𝘃𝗶𝗰𝗲 𝗮𝗿𝗿𝗮𝗻𝗴𝗲𝗺𝗲𝗻𝘁𝘀 – Clinic location, operating hours, appointment procedures, patient registration, and arrangements for accommodating eligible SLSU personnel.
▪️𝗕𝗲𝗻𝗲𝗳𝗶𝗰𝗶𝗮𝗿𝘆 𝗿𝗲𝗴𝗶𝘀𝘁𝗿𝗮𝘁𝗶𝗼𝗻 𝗮𝗻𝗱 𝗰𝗼𝗼𝗿𝗱𝗶𝗻𝗮𝘁𝗶𝗼𝗻 – Procedures for assisting eligible members with registration, verification, and availment of covered services.
▪️𝗤𝘂𝗮𝗹𝗶𝘁𝘆 𝗮𝘀𝘀𝘂𝗿𝗮𝗻𝗰𝗲 𝗮𝗻𝗱 𝗿𝗲𝗽𝗼𝗿𝘁𝗶𝗻𝗴 – Proposed mechanisms for monitoring service delivery, addressing patient concerns, and coordinating with the University and relevant PhilHealth offices.
▪️𝗔𝗱𝗱𝗶𝘁𝗶𝗼𝗻𝗮𝗹 𝘀𝗲𝗿𝘃𝗶𝗰𝗲𝘀 𝗮𝗻𝗱 𝘀𝘂𝗽𝗽𝗼𝗿𝘁 – Other relevant services, health education activities, and preventive healthcare initiatives that may complement the YAKAP Program, subject to applicable PhilHealth rules.
All proposed services must comply with PhilHealth's prevailing policies, benefit coverage, provider accreditation requirements, and other applicable regulations. Any proposed supplemental services or arrangements must be clearly identified and distinguished from benefits covered by PhilHealth.
📍𝗘𝘃𝗮𝗹𝘂𝗮𝘁𝗶𝗼𝗻 𝗼𝗳 𝗣𝗿𝗼𝗽𝗼𝘀𝗮𝗹𝘀
All submissions shall be subject to review and evaluation by a duly constituted committee based on established criteria, including accreditation and regulatory compliance, service capability, accessibility, adequacy of facilities and personnel, responsiveness to beneficiaries’ needs, and overall capacity to support the effective implementation of the PhilHealth YAKAP Program.
The evaluation shall be conducted in accordance with applicable laws, rules, and regulations to promote transparency, fairness, and accountability in the selection process.

Incomplete submissions or proposals that do not satisfy the applicable eligibility requirements may be subject to disqualification.
