(1) The top section of the form contains your basic student information, including your full name, course, school year and semester of enrollment, student ID number, and year level.
(2) Directly below this is your class schedule matrix, which provides a detailed overview of your enrolled subjects. Each column contains the following information:
Column 2: Subject Code
Column 3: Section
Column 4: Subject Description
Columns 5 to 7: Number of Units (Lecture, Lab, Total)
Column 8: Class Schedule (Time)
Column 9: Class Dates (based on your assigned shift)
Column 10: Room or Venue of Classes
(2.1) In Column 8, the class schedule before the slash (/) corresponds to the class date before the slash in Column 9. Likewise, the schedule after the slash in Column 8 matches the class date after the slash in Column 9.
Beneath the schedule is the breakdown of your school fees:
(3) On the bottom left, youโll find the breakdown of tuition and miscellaneous fees, as well as other school fees such as uniforms, learning materials, insurance, and more.
(4) On the bottom right, you'll see any applicable tuition discounts, the total amount due, and the installment schedule per payment period.
