✅ Two (2) Recommendation Letters from a former professor, immediate supervisor, or research adviser
✅ Result of DNSC Graduate School Admission Test (GSAT)
✅ Latest copy of Official Transcript of Records (TOR) for evaluation purposes
✅ Certificate of Honorable Dismissal from the last school attended
✅ PSA or NSO Birth Certificate with a clear copy of the original issued by the Local Civil Registrar
